The Center for Health Education, Medicine, & Dentistry (CHEMED) is a Federally Qualified Health Center located in Lakewood, New Jersey. Since 2008, CHEMED has offered primary and specialty care services, as well as patient education, to its primarily Orthodox Jewish and Hispanic patient base. Currently located at two sites in Northern and Western Lakewood, CHEMED will be opening a center on Lakewood’s Southern border, as well as a location within the Belz community, so as to ensure that all patients have easier access to care. CHEMED’s integrated care model allows patients to seamlessly receive a wide range of services including Radiology, Women’s Health, Behavioral Health, Podiatry and Dentistry all in one location. Patients are screened at their primary care visits for Behavioral Health and Substance Abuse concerns, and are referred to the Behavioral Health Department when needed. Brief interventions are performed in the exam room as well.

In addition to regularly scheduled appointments with their providers, patients can walk in to CHEMED’s Fast Track department for matters that need immediate attention.  In addition, CHEMED has a high acuity urgent care center, allowing patients who do not require an emergency room visit to receive quick, quality service close to home.   CHEMED’s Care Management Department works with the most medically complex patients to manage patient compliance and stick to the provider’s care plan, improving patient outcomes.

CHEMED’s Administrative staff works persistently to keep up with internal patient growth as well as the future of medicine, providing the services and technology that today’s patients need, while always maintaining the ultimate goal of high quality care. Creating a think tank on Jewish medical/ethical questions and ideas enables the comprehensive network of superior care within the medical community to continue to expand and further advance these shared goals.
